MPAC RESERVATION REQUEST
THIS IS A REQUEST FOR A SERVICE AGREEMENT BETWEEN THE UNIVERSITY OF SOUTHERN MISSISSIPPI, SCHOOL OF MUSIC, MANNONI PERFORMING ARTS CENTER AND TO ENSURE THE EXPECTATIONS OF BOTH PARTIES ARE ACCEPTED AND AGREED UPON BEFORE THE AFOREMENTIONED EVENT TAKES PLACE.
Upon receipt of this completed application, the event manager will contact you
to confirm your reservation and to discuss equipment and staffing.
Be sure to read the MPAC Guidelines before completing this application. They will become part of your contract and will be enforced. You may find them here http://www.usm.edu/music/facilities.
Events are not confirmed until you have a completely executed service agreement and reply confirmation email from the event office.
DO NOT ADVERTISE YOUR EVENT UNTIL YOU HAVE RECEIVED CONFIRMATION FROM EVENT COORDINATOR.
