This form is for registered student organizations only at the HATTIESBURG CAMPUS. Outside groups, offices and departments should contact the building manager of the location you are requesting for further assistance.


Complete this form to request your proposed event or meeting if, ONLY if, your organization is a registered student organization on campus. The organization assumes the responsibility for its activities, meetings, and/or events. All student organizations are required to follow rules set forth in the Policy, Procedures, and Guidelines manual which can be found on our website. Please contact our office ahead of time if you have any questions about following policy.

Forms are due Monday at 4pm for meetings and small events the following Sunday-Saturday. If Monday is a holiday, forms are due the Friday prior at 4pm. Special events require at least one months notice.

The form will not allow you to select a date after the deadline has been met. Be sure to plan ahead as an organization to ensure you meet the deadline each time or you will have to adjust your plans accordingly. This means NO late forms are accepted.

Use this form to request space for ALL STUDENT ORGANIZATION ACTIVITIES. Be as accurate as possible when providing information concerning your event. Remember, all information listed on the form should match the information we have on file from your Organization Information Form (OIF). *The Office of Leadership and Involvement reserves the right to deny requests if contact names and information do not match.
