Greek Life Request Form 2012-2013
Forms are to be submitted by Mondays at 4 p.m. for events the following Monday through Friday. Special Events require forms be submitted at least ONE MONTH in advance.
1. You will be contacted at the email you provide on this form about attending a pre-event meeting one month in advance of your event.
2. All pre-event meetings are on Tuesday starting at 9:45 a.m.
3. You will have to attend a pre-event meeting for your event to be registered with the Office of Student Activities.
All gatherings/house parties are now defined as a HOUSE PARTY. UPD will determine cost based on number of parties that night. House parties are only allowed on Thursday, Friday and Saturday nights unless prior approval is given by the Student Activities committee for a special circumstance.
Remember you are responsible for paying UPD in correct change and CASH no later than TWO WEEKS prior to your event. Failure to do so could result in your event being cancelled or not registered.
Payne Center Events
**All events in the Payne Center require a one-on-one meeting with Payne Center staff before your event is approved. YOU MUST set this up with them yourself after you submit this form.
**If you select the incorrect ype of event you are hosting The Office of Student Actitivies or The Office of Greek Life has the authority to change the event type.
Please remember your event will not be registered until you receive notification of approval from the Office of Student Activities via email.
